Manual Handling Incident Investigation

The reason for investigating an incident or near miss is to determine; the cause or causes of the incident; to identify any present or future hazards, safety system failures or faulty or non-optimum work procedures that contributed to the incident; and to recommend corrective action to prevent similar incidents.

Incidents should be investigated by people who are knowledgeable about the type of work involved at the time of the incident. Health Safety Representatives and relevant workers should also be involved in the investigation. An incident /near miss investigation report should answer the WHO, WHERE, WHEN, WHAT, WHY and HOW questions with regard to an incident.

We will work with you to determine an outcome that will change your safety system for maximum benefit. There are a number of methods that can be used to assist the investigation. Selecting the right method will be determined subject to the initial data supplied. ICAM and Taproot are preferred incident investigation tools.

Outcome

Upon completion of this audit, a full written report will be generated highlighting the issues to be discussed in the follow-up meeting. We are able to assist in implementing changes and carrying out supporting training.
